Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“It was determined throughout the course of the HHS Investigation that a 2-year-old child in care was left alone unsupervised inside the enclosed playground area for a duration of 3 minutes and 52 seconds.”
“It was determined throughout the course of the DFPS Investigation that a person in charge did not have an eligible background check and had access to young children in care.”
“It was determined throughout the course of the DFPS Investigation that a young child in care was left alone unsupervised by a caregiver for approximately 15 minutes.”
“While surveying personnel records, it was determined that 4 of the 6 records did not have proof of GED/High School Diploma/or Equivalent.”
“During the inspection the last CPSC annual certification was dated back to 2020. There wasn't a current CPSC annual certification posted. This was immediately corrected during the inspection, placing the operation back into compliance.”
“During the inspection some caregiver files were missing the required Annual Health and Safety Audit training and certificates.”
“During the inspection, disinfectant was located in 2 classrooms by the sinks, accessible to the children in care. Chemicals located underneath the sink in the kitchen were also not secured with child safety locks and accessible for children in care. This was immediately corrected by locating the disinfectant in high cabinets and fixing the locking mechanism on the door under the kitchen sink.”
“During the inspection the permit renewal was not posted with the required postings. This was immediately corrected during the inspection, placing the operation back into compliance.”
“During the inspection 2 of the 4 children's files surveyed had outdated immunization records. This was immediatly corrected by printing the most current IMMTRAC2 record available, placing the operation back into compliance.”
