Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Seven of the 10 files evaluated do not have pre employment affidavits. The employees have started after October, 2023.”
“A caregiver accidentally left a child alone on the playground.”
“During the evaluation of children's records, it was found that one child had an expired Teva Epinepherine Auto Injector.”
“A child was left behind in the restroom during transitioning to PE.”
“The caregivers left the classroom door open and a child eloped.”
“One child enrolled does not have a physician's signature on their FARE form.”
“The operation does not have a current sanitation inspection.”
“A child was bitten multiple times by another child while the caregiver changed a diaper. Due to the layout of the toddler/two-year-old classroom, an additional caregiver in the classroom during diaper changes was also necessary for adequate supervision.”
“A caregiver failed to assess a child for injury after two children were separated following an altercation”
“A ball located on the infant- toddler playground was torn with exposed foam. This was corrected when the director had the ball removed from the playground.”
“A teacher did not wash her hands after she changed a diaper.”
“An infants hands were not washed after they had their diaper changed.”
“The provider did not report the number of employees by the due date.”
“There is no documentation showing fire extinguishers have been tested monthly.”
“There is no documentation showing smoke detectors have been tested monthly.”
“There is no carbon carbon monoxide detector.”
