Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“When reviewing staff files, the annual training on abuse and neglect was not available for review.”
“When reviewing staff files, one staff member did not have the required abuse and neglect training available for review.”
“One out of three staff members did not have pre-service training available for review.”
“A site director has not yet been assigned to this location, and the necessary paperwork has not been provided despite multiple requests.”
“Re-citing: One caregiver reported having 14 children in her group, after counting multiple time she had 13 in children in her care. The second caregiver mentioned she had 17 children in care, after counting multiple times, the amount of children present in her group was 16. Neither caregiver stated the correct amount of children in there care.”
“One caregiver reported having 18 children in her group, while a second caregiver mentioned she had 20 children. The third caregiver indicated that there were 15 children in her group. Initially, this totaled 53 children. However, after counting several times, I found that there were only 48 children present. The person in charge insisted there were 53 children, but the caregivers then recalled that some children had left for the day, which led to a revised total of 48 children.”
“Three out of the four staff files reviewed did not have the required topics in health and safety available for review for the previous training year. Note: This was corrected at inspection when health and safety trainings were added from the current training year which cannot be counted again for a different training year.”
