Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The lights in the toddler room were turned off and the window was covered during naptime. There was insufficient light to have a clear view of the room. Note: During inspection provider removed the window cover allowing light into the room.”
“Last fire drill, CO detector and Fire Extinguisher checks were documented on 05/19/2025. Per provider a fire drill, CO detector and Fire Extinguisher checks were conducted on 06/23/2025 but not documented. Note: This was corrected at inspection when provider documented the drill and checks.”
“There was no translation documentation available for a caregiver's education documents written in a foreign language.”
“There was no smoke deterctor in the after-school room. Note: This was CORRECTED during the inspection as a battery-operated smoke detector was installed in the room.”
“The director was missing annual training in the following topics: (1) Preventing and controlling the spread of communicable diseases, including immunizations; and (2) Handling, storing, and disposing of hazardous materials (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).”
“The operational policies reviewed were missing the following information: (A) The benefits of physical activity and outdoor play; (B) The duration of physical activity at your operation, both indoor and outdoor; (C) The type of physical activity (structured and unstructured) that children may engage in at your operation; (D) Each setting in which your physical activity program will take place; (E) The recommended clothing and footwear that will allow a child to participate freely and safely in physical activities; and (F) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play.”
“A smoke detector was missing in the infant room. Note: This was CORRECTED during the inspection as a battery operated smoke detector was installed in the room.”
“2 of 4 children's files reviewed did not have the most current immunization record available. Note: A copy of the current immunization record for one of the two children was obtained during the inspection.”
