Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The written operational policies were incomplete for the local licensing address and phone number, suspension and termination policy, parents rights, and the facility's open door policy.”
“3 of the children's records evaluated did not have the medical authorization and physician's information. 1 file was observed to have a possible allergy, but no allergy plan was on file.”
“During the inspection, a substitute caregiver did not have signatures on the application. This was corrected when the caregiver signed the form.”
“A child under the age of two was observed watching screen time activities. This was corrected during the inspection when the provider turned the television off.”
“During the inspection, the operational policies were incomplete for animals, release of children, outdoor activities, and emergency preparedness.”
“A box of cat liter was observed in unsanitary conditions while children were in care. This was corrected during the inspection when the provider emptied and cleaned the box.”
“The operation was found to not have the CPSC form for the current year. This was corrected at inspection when the operation completed and posted the required form.”
“It was found the operation was not documenting the lock down drills after being completed. This was corrected at inspection when the drills were documented on the form”
“Fire extinguisher was found to not be mounted.”
“It was found the operation was not documenting the sheltering drills after being completed. This was corrected at inspection when the drills were documented on the form.”
“Television was observed to be on and a child under the age of two was present. This was corrected at inspection when the television was turned off.”
“Cabinets located in the bathroom used by children in care were found to be easy to open and had razors, toothpaste, and cleaning materials in them. This was corrected at inspection when the items were removed from the bathroom cabinets.”
“1 children's file from 2 reviewed was found to not have a health statement, the child has been in care at the operation for more than 12 months.”
