Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“An infant was in a bouncer for over fifteen minutes.”
“A bottle was propped on a blanket in the infant room.”
“One staff in charge of transportation did not have completed annual transportation training. Note: completed during the inspection time.”
“One child was missing feeding instructions information.”
“Promotion of indoor and outdoor physical activity policy was incomplete.”
“Fire extinguishers were last inspected October 2023 and require annual inspections. Annual inspection is past due.”
“A review of the Employees Who Left Employment in 2024 data revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.”
“At follow up, there was not a fire drill conducted in February 2024.”
“There has not been a qualified Director at the center since November 2023. Since a qualified Director has not been in charge of the center the operation has been cited for emergency practices that have been overlooked. Assistance was requested by center staff for emergency drills. The fire drill was not conducted because management did not assist the center staff.”
“Lockdown drills were not conducted 2 times in 2023.”
“The school age playground had a lot of trash accumulated on the side in the grass. The school age child was kicking the trash during inspection.”
“The fire inspection was last conducted 2/7/2023 and is now expired.”

“A caregiver was present without a background check,”
“During inspection of the classrooms, I observed four occupied classrooms with dirty, unsanitary areas that were accessible to children. These areas included walls behind the trash where the trash can is, the bathrooms and toilets, and floor area near diaper changing table in the 2's class. The white covering on the particle board beneath the handwashing sink in the school age classroom is breaking and chipping.”
“A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.”
