Source: Delaware Office of Child Care LicensingRecords held from Sep 27, 2021 onward. Earlier visits may exist in the agency’s own files.
Delaware publishes only visits where a regulation was cited, plus complaint investigations. Visits that found no violations do not appear in the state's data, so a gap between dates here does not mean the program went uninspected. Delaware does not publish a severity weight for its citations.

Violations
Text reproduced verbatim · Delaware Office of Child Care LicensingNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.

“On January 31, 2024, a complaint was called into the Child Abuse and Neglect Report Line and not accepted for investigation by the Institutional Abuse Unit (IAU). The Office of Child Care Licensing received the complaint on January 31, 2024, and accepted it for investigation regarding lack of supervision. While the administrator had self-reported the incident, the complaint is substantiated for lack of supervision because OCCL received a complaint regarding this incident and video footage showed a child running out of the building and into a multi-lane road. Staff interviewed also stated they were unaware the child was not in the classroom or building until police officers returned the child six minutes later. An additional concern noted was the classroom the child was in was over proper staff-to-child ratio based on the age of the youngest child present.”
“The Office of Child Care Licensing (OCCL) received a complaint on December 19, 2023, regarding not conducting monthly fire drills, staff being told to give false information to OCCL, unqualified staff, insect infestation, sanitation concerns, and lack of a security procedure for entering the facility. An investigation was conducted on January 4, 2024. The complaint is substantiated for leaving unqualified staff alone with children. According to documentation provided, there were three instances where an unqualified staff member was left alone with children between December 7, 2023, and December 12, 2023. According to documentation provided, another concern noted was the infant room was left out of correct staff-to-child ratio for a total of one hour and twelve minutes and six infants were unsupervised for three minutes the same day. There is no evidence to substantiate this complaint for not conducting fire drills because according to documentation provided and staff interviews, fire drills are conducted every month as required. There is no evidence to substantiate this complaint for staff being told to give false information to OCCL because according to staff interviews, they were never told to give false information to OCCL. There is no evidence to substantiate insect infestation as there were no insects present in the center at the time of the complaint, and based on documentation and staff interviews, a pest control company services the center at least once a month. There is no evidence to substantiate sanitation concerns because based on staff interviews and observations, trash is taken out consistently and the center is cleaned daily. There is no evidence to substantiate a lack of security procedures for entering the facility because according to staff interviews and observations, there is a security procedure in place for entering the facility for staff, parents, and visitors.”
“All child files must be complete. -A blood lead test result is needed for two children.”
“The Office of Child Care Licensing received a complaint on April 4, 2023, regarding staff-to-child ratios, lack of supervision, child release policies, building security, sanitation, diapering procedures, and procedures for handling parent complaints. An investigation was conducted, beginning on April 26, 2023, and ending on May 1, 2023. The complaint is substantiated for building security because no one was monitoring the entrance to the secondary building and the door was unlocked. There is no evidence to substantiate the complaint for lack of supervision because all staff stated they never leave children unsupervised and children were appropriately supervised at the time of the visit. There is no evidence to substantiate the complaint for incorrect staff-to-child ratios because observations at the time of the visits and attendance tracking documents reviewed showed classrooms to be in correct staff-to-child ratio. There is no evidence to substantiate the complaint for failure to follow the correct release of children policy because all staff knew the proper way to verify release for children and all staff involved in the complaint incident reported they followed protocols for release of children and checking the proof of identification covered in the center s policies. Relative #1 had left the center with Child #1 while staff attempted to get verification of her identification. There is no evidence to substantiate the complaint for sanitation issues because the entire center was clean at the time of the investigation, and the center did not smell of dirty diapers. There is no evidence to substantiate the complaint for not following procedures to handle parents complaints because assistant directors both denied receiving complaints from Parent #1 about the alleged issues.”
“A complaint was received on September 9, 2022, regarding not following security procedures, not verifying a child s identity when a child not known to the staff person arrives, and not verifying a child s food allergies prior to serving snack. An investigation was conducted on September 12, 2022. The complaint is substantiated for not following security procedures, because Parent #1, a stranger to the facility, was able to enter and walk through the hallway where she had access to children. The complaint is substantiated for not verifying a child s identity, because Staff #1 admitted to asking the unknown child her name and writing it on the attendance record without verifying whether the child was enrolled at the facility. The complaint is substantiated for not verifying whether a child had allergies prior to serving the child a food to which the child is allergic, because Staff #1 admitted she did not check to see if Child #1 had any food allergies.”
“All staff files must be complete. Staff #1 needs 7 hour Health & safety for children child care professionals certificate in lieu of Administration of Medication certification.”
“All child files must be complete. Child #4 needs lead blood test results.”
