Violations
Text reproduced verbatim · Ohio Department of Children and YouthNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.

The following was observed: the weather and fire alert plan was not posted in the downstairs bedroom. Correct the violation and submit the program's corrective action plan to verify compliance with the requirement of the rule.”
“During the inspection, it was determined the program’s written emergency preparedness and response plan did not meet the requirement or was missing the information. The following was observed missing: the program's tornado policy stated children would shelter in basement, the home does not have a basement; the EPRP did not state how parents are provided with the provider's contact information; the procedures for notifying/communicating with parents regarding the location of children if evacuated did not state the parents or emergency contacts would be called; and throughout the document, references were made to staff/CCSM, the provider is the only staff. Submit the program’s corrective action plan, which includes the missing information, if applicable, to verify compliance with the requirements of this rule.”
“In review of the program's records, it was determined that requirements for written permission from the parent/guardian for a field trip or routine trip were not met. The following was observed: routine trip forms were not updated yearly. Submit the program’s corrective action plan to verify compliance with the requirements of this rule.”
“On the day of the inspection, the program’s written policies and procedures provided to the parents/guardians and employees was missing item(s). The following was observed as missing/outdated: In staff/child ratios and group size, the maximum capacity needs changed to 7. In the payment section, the rates do not match the rates listed on OCLQS. In the enrollment section, it states medical forms are required for all children. This is no longer the case. Children not yet attending school must have a immunization record on file within 30 days of attendance. was In the Suspension, Expulsion, and Disenrollment section, the policy does not state that the provider will create and sign a written behavioral plan with parents before expelling a child due to behavioral issues as required by rule 5180:2-13-19(C)(2). In the food and dietary section, it refers to four basic food groups. This needs updated to 5 basic food groups to align with rule (protein, dairy, vegetables, fruit, and grain). Throughout the document, mentions of staff/CCSM need removed as the provider is the only staff. The section regarding medication and topical products needs to update the name of the form required. It is now DCY 01236 "Health Care Plan Documentation & Permission to Administer Medication". The policy regarding the usage of emergency medical devices and emergency medications needs redone. This policy is referring to if the program has emergency devices/medication on hand that could be used for anyone. I.e. AED, naloxone, anti-choking device, etc. This policy needs to state whether or not the program has such devices. For example, "My program does not use emergency medical devices and medications such as AED and naloxone. The Written Information needs to include policies regarding evening and overnight care (7pm-6am). This should include that the provider will remain awake until all children are asleep; how the home with provide adequate lighting so provider can see children, and parents can see safely during pick up and drop off; children under the age of 5 sleep on the same floor with the provider; bedtime routines followed; children's pjs/toothbrushes/personal items kept separate; and bathing children while ensuring school age boys and girls are separated. Please refer to rule 5180:2-13- 21 and pages 86-88 for more information regarding evening/overnight care. Correct the violation and submit the program's corrective action plan to verify compliance with the requirement of the rule.”
