Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One out of four student files were missing medical authorization.”
“One out of four files are missing updated immunizations.”
“Two out of four records lacked a health statement from a physician.”
“The caregiver did not wash the child's hands following a diaper change.”
“Caregiver did not wash hand after changing the child's diaper.”
“Two caregivers' records were unavailable during the inspection.”
“There were no paper towels in the children's restroom.”
“The sheets were not snug in the cribs.”
“Two infants were observed to have been left seated in their highchairs for over 15 minutes.”
“The last fire drill was performed three months ago.”
“The license or registration certificate for the child-care home was not posted.”
“The room was dark enough that my eyes had to adjust.”
“The caregivers failed to inspect the playground for the presence of hazardous materials or objects.”
“Bugs have been found in the stagnant water at the children's playground.”
“Despite the pleasant weather, the children stayed indoors all day.”
“Two caregivers lack training in preventing shaken baby syndrome, Sudden Infant Death (SID), and abusive head trauma.”
“There were no children's records available for review.”
“Caregiver did not notify parents that the children will be going to the park the day.”
