Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A caregiver used profane language in the presence of children in care.”
“A caregiver utilized yelling in the classroom.”
“A caregiver threw folders at a child striking the child in the face.”
“A caregiver utilized yelling in the classroom.”
“Based on information gathered during this investigation, staff failed to count the children they were responsible for resulting in a child being able to exit the operation unsupervised. Staff was unaware the child was missing for approximately 15-20 minutes until they were contacted by law enforcement.”
“Based on information gathered during this investigation, staff did not demonstrate good judgement because caregivers were aware of a child's tendency to elope and required increased supervision. Despite the known behavior, staff did not take the needed precautions to ensure the child's safety and measures were not implemented to prevent the child from exiting the gate.”
“Based on information gathered during this investigation, a child was able to exit the operation without supervision. The child walked down the street and was eventually taken to the police department by a passerby. The child was gone for an extended period of time.”
“3 out of 8 children's records evaluated had out of date or missing immunization records.”
“1 out of 4 employee records evaluated was missing a notarized employment affidavit.”
“During the investigation it was found that the daily report provided to the parent did not include all the updated feedings and diaper changes that occurred on that particular day.”
“The fire extingushers lacked the annual servicing.”
“The operation lacked a current fire drill. CI: This was corrected at inspection when a fire drill was conducted during the inspection.”
“One out of ten children files lacked a DTAP immunization.”
