Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One out of eight employee records evaluated were lacking training in child maltreatment.”
“Three out of eight employee records evaluated were lacking a notarized employment affidavit.”
“Two out of eight child records evaluated were lacking a health statment signed by a medical professional.”
“The director was lacking trainings in the topics of child maltreatment, SIDS, brain development, emergency preparedness, building safety, food allergies, etc.”
“Documentation for emergency drills was not available for review.”
“The operation has not had a health inspection in over 12 months.”
“The operation has not had a designated director since September 2024.”
“Staff did not practice good judgement when they addressed issues amongst each other in a classroom with children present. Voices became elevated during this incident.”
“A witness observed an employee disciplining their own child utilizing prohibited punishment while the child was in the care of the operation.”
“There have been multiple incidents in which staff have stepped out of their classroom leaving children unsupervised during naptime.”
“Two caregivers did not wash their hands after removing gloves after changing a diaper. This was corrected at inspection when it was addressed with the caregivers.”
“3 out of 5 classrooms observed had out of date activity plans posted.”
“4 out of 4 employee records were missing the pre-employment affidavit.”
“In the toddler room, diapers were observed in an uncovered container on the floor. In the Pre-K classroom wipies and diapers were observed stacked on a chair. In both classrooms the items were in reach of the children.”
“Several children were observed eating crackers and cheese sticks on a bare table.”
