Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“2 children did not have signatures for emergency medical authorization.”
“One child did not have an immunization record on file.”
“Live roaches were seen under a portable handwashing sink.”
“There was not documentation for the last two months for fire drills.”
“The loose-fill present on the active play area is not being maintained to the proper depth and is covered by grass.”
“A child was observed asleep in swing.”
“An infant under 12 months of age was asleep in a crib with a pacifer clip attached to them while asleep.”
“There is one out of four vehicles that is not equipped with the child safety alarm that is currently transporting children.”
“There is one out of eleven children records that does not have a current immunization on file.”
“There were two classrooms in the facility that did not have the evacuation relocation posted on the classroom. Note: This was corrected at inspection when the director posted the fire evacuation plan in both classrooms.”
“Children were observed to not have water made avaliable to them during their outside time.”
“There was a hole in the wall by a toddler table accessible to children that left sheet rock exposed. There is also peeling paint down low enough to be accessible to children.”
“The operation was unable to provide a copy of their transportation emergency plan.”
“A caregiver did not know the correct number of children in their classroom.”
“The school age classroom was out of ratio. There were 30 children in the classroom with a SAG of 6 to 8 years old. * This was corrected at inspection by moving 4 children to a different classroom. **”
“The caregiver inside the school age classroom did not know the correct number of children in their care. ** Teacher was informed of her correct count while at inspection.”
“A infant was left unattended inside a classroom. 3 other children were also left unattended in another classroom.”
“One person present in infant room during inspection did not have a clear background check.”
“During my arrival I saw a 5 year old child walked into the center from a transportation van without a caregiver into the center lobby. There was no one in the lobby. There were many times school age children were seen in the hallways without caregivers in the hallways with them.”
“During inspection, 6 school age children walked into infant classroom with 10 infants present. The youngest infant was 4 months old.”
“During walk through the infant room had 10 infants ranging in age from 4 months to 22 months. Note: A second caregiver did not come in the classroom for approximately 15 minutes.”
“A 4 month infant had a soft toy book in their arms while sleeping. Inspector removed toy from crib.”
“The director failed to notify parents that an 8 month old and an 18 month old infant were left alone on March 23rd for 1.5 minutes, where the older infant scratched up the 8 month old infant's face. The director did not help caregivers maintain compliance with proper supervision of children. Children were allowed to walk out of the classroom without supervision.”
“Based on evidence collected during the investigation, all parents were not given access to entire center until March 29, 2022.”
“Based on evidence collected during investigation, on March 23rd video footage shows that 2 infants, ages 8 and 18 months were left alone in a classroom for 1 1/2 minutes. The caregiver walked out of the class and then walked back by class 30 seconds later without regard.”
“The mulch surfacing on the playground with the slides is very thin. It appears to be an inch in depth. The side platform on this playground is over 5 1/2 feet tall.”
“The toddler classroom had 5 bottles/cups not labeled with names of the children it belonged to.”
“The two slides located on the playground have 4 bolts sticking out of pipes that support the platform of the top of the slide. Each of the bolts stick out approximately 1/2 inch. The bolts are 5 feet off the ground and could catch a child's clothing. A child could also run into these bolts sticking out causing injury.”
