Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Three infant caregiver files that were audited did not have SIDS/Shaken Baby/Early Brain Development for the previous training year.”
“The toddler room was observed serving food on a bare group table setting with no individual plates/bowls.”
“The pre k room had some nap mats that were torn exposing the padding. This was corrected when the mats were disposed of and replaced. The pre k playground had a few broken toys that were sharp to the touch. This was corrected when the toys were disposed of.”
“The operation staff failed to ensure that all children were accounted for while transitioning them back into the building resulting in two children getting out the playground due to the gate being left open and being found over 200 feet away from the center close to a busy street. The two children were unsupervised for approximately 2 minutes before being found and taken back to the operation. The operation terminated the caregivers responsible for the incident as soon as it was discovered.”
“This standard was found deficient as part of a DFPS Investigation.”
“It was determined that the parents of children involved in an incident that placed them at risk were notified of the incident, but the operation did not obtain signatures from the parents on incident report forms completed.”
“Two plastic containers in the school-age room that were accessible to children were cracked and sharp to the touch. This was corrected when the containers were thrown away.”
“The operation was missing operational policies involving criteria to determine extreme weather conditions, plan to ensure physical activity occurs on extreme weather days and procedures for supporting inclusive services. This was corrected during the inspection visit by the director.”
“The toddler playground had a few broken toys that were sharp to the touch and accessible to children. This was corrected when the toys were removed from the playground.”
“Cleaning supplies were found under the cabinet in the PreK bathroom where there was no lock and accessible to children. This was CORRECTED when the staff put a lock on the cabinet.”
“Eight written feeding instructions were not updated within the 30 days.”
