Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A child was left unsupervised on the playground.”
“I did not observe an eligible background check status for a caregiver who was working with children. NOTE: The caregiver left during the inspection.”
“I observed an emergency food allergy action plan that was not signed by the child's parent. NOTE: This was corrected during the inspection.”
“I did not observe information concerning the center's policies on active play documented in the operational policies.”
“The number of employees who had left their employment in 2021 was not reported to HHSC - Child Care Regulation by the 1-15-22 deadline. NOTE: This deficiency has been corrected.”
“I evaluated 10 children's records and observed the following: 1. No address documented for the emergency contact in 2 children's records. 2. No date of admission in 1 chiild's file. 3. No information indicating who the parents have authorized to pick up the child in 1 child's file. 4. No information indicating whether the child has special needs documented in 6 out of 10 children's records evaluated.”
“I observed one caregiver supervising 6 children. The median age of the group was 15 months and the required child/caregiver ratio for 12-17 months is one caregiver for every 5 children.”
