Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Activity plan was not posted in school age classroom. Activity plans in toddler and infant classroom did not outline specific daily activities.”
“Operational policies were missing information on suspension and expulsion of children, immunizations, instructions on how to access HHSC website, abuse and neglect website and online minimum standards.”
“One toilet at the operation has a brown/black ring in its bowl.”
“The operation did not have an extra employee in the building during naptime. There were three employees. Two employees were counted in child/caregiver ratio in the infant classroom, and one employee had a combined group. Infant classroom was out of child/caregiver ratio when one caregiver walked out of the room, to get a roll of towel paper.”
“Emergency evacuation and relocation diagram did not include the floorplan for the whole center. It was missing information on the location inside the center where where all caregivers and children take shelter from threatening weather, and the location outside the center where all caregivers and children meet to ensure everyone has exited the child - care center safely”
“Emergency preparedness plan was not available for review.”
“The operation has a step-2 Clubhouse Climber in the outside playground. The designated play surface has a height of 23 inches. The operation has turf grass, under the equipment.”
