A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“All 4 caregivers were missing specific topics of their required annual trainings.”
“The operation had two hanging rings for children to swing on that were installed on trees. The design, scale, and location of the equipment was not used according to the manufacturer's instructions as the devices appear to be homemade. This was corrected when the provider removed the two rings during the inspection.”
“Three caregivers did not have training in handling, storing, and disposing of hazardous materials.”
