A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“An infant was sleeping in a Boppy nursing pillow that was inside a crib; The operation did not have an Infant Sleep Exception/Health Care Professional Recommendation Form (3019) with a signed statement from a health-care professional that the alternate position is medically necessary.”
“An infant was laying on a Boppy nursing pillow, feeding himself without caregiver assistance or direct supervision.”
“A caregiver was observed using a cellphone while attending to children.”
