Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Four of six staff were lacking training in "Preventing, Controlling, Responding to emergencies due to food or allergic reactions and "administering medication". A list of staff were left with director.”
“It was observed that several areas on the playground that require loosefill were lacking the adequate depth required.”
“It was observed during the walkthrough there were rotted wood on outside on play equipment and playground door frame, there were several peices of metal outside play equipment with chipped paint exposing rusted metal, and a tree root was present next to play equipment that could cause a tripping hazard.”
“The children's records evaluated lacked documentation that the parents received a copy of the parents rights. The operational policies also lacked the HHSC website (www.hhs.texas.gov); information on whether or not employees are required to obtain vaccines for preventable diseases; and procedures for supporting inclusive services to children with special care needs.”
“A sampling of children records were evaluated. One child's record lacked a health statement. One child's immunization record was observed incomplete. Provided a list of the children and their missing information to the director at inspection.”
“One volunteers record lacked a notarized Affidavit for Applicants for Employment with a Licensed Operation or Registered Child-Care Home (Form 2985).”
“The Emergency Preparedness Plan was evaluated and lacked written procedures for: How your staff will continue to care for the children until each child has been released.”
