Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Four trash bags containing soiled diapers and other unsanitary items were found located outside doorways and near the playground area. There was also other trash debris around the doors.”
“A storage closet containing cleaning supplies was unlocked. The locking mechanism was no longer working. This closet was located in a hallway among classrooms and easily accessible to children passing by.”
“Two classrooms with children between the ages of 12-24 months were found to have children sitting in activity tables without the use of safety straps. It was found that the majority of seats did not have a safety strap attached.”
“A restroom used by children had a trashcan containing soiled diapers/pull-ups with no lid. The diapers/pullups were not bagged separately.”
“Two children were observed with their hands being left unwashed; one after a diaper change and one that was potty-trained.”
“A five month old infant was observed to be sitting in a swing holding a bottle of milk.”
“The operation had a caregiver in a classroom supervising children that did not have a current background and fingerprint check. This was corrected at inspection by removing the caregiver from the operation.”
“One of the outdoor play areas licensed for school aged children was observed to have a wire rope attached between 2 poles causing a possible strangulation hazard. This was corrected at the inspection when the wire rope was removed and thrown away. A toilet used by school age children was observed to be out of order and covered with a trash bag. The trash bag contained feces on the outside. This was corrected at inspection when the soiled trash bag was removed and new one was placed over the out of order toilet. Tape was also placed across the stall entray way preventing entrance into the stall. A trash bag of dirty diapers was observed on the 2 year old playground. The trash bag was torn exposing dirty diapers. This was corrected at inspection when the trash bag was removed from the playground.”
“A school age child was observed to be unsupervised while going to the bathroom. This was corrected at the inspection by having the caregiver stand in the door way in order to see all children on the playground and in the bathroom.”
“A 12-18 month old classroom was observed to have one caregiver with 6 children during naptime. This was corrected at inspection when one of the children was moved to another classroom with 2 caregivers.”
