Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During inspection, one out of the ten files reviewed. One staff was missing 3.5 hours of annual training for 2024-2025.”
“Based on information obtained during the DFPS investigation, a caregiver used poor judgment and demonstrated a lack of self-control by inappropriately touching a child in care.”
“Based on information obtained during the DFPS investigation, the director was aware that a caregiver showed favoritism towards a child, and a child did not have the same consequences as the other children in a classroom.”
“This standard was found deficient as part of a DFPS Investigation. There is sufficient evidence to support the allegation of sexual abuse.”
“Based on the information obtained during the investigation inspection the operation admitted to a child being left alone in the classroom unsupervised.”
“During inspection staff hired on 6/12/2023 was observed supervising children without background check results.”
“Based on interviews conducted staff were not using safety straps when placing children in highchairs.”
“Staff hired on 8/22/2018 had 22 hours of annual training for the 2021-2022 year. Staff needs 2 hours.”
