Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Sanitary holder was not available for one child who was eating snack on the table. This was corrected when the caregiver provided the child with a napkin.”
“Based on the information provided, the parents were not adequately notified during pick up about an incident/injury their child sustained while in care. This was corrected when the parent reached out to the school later that day.”
“One child's immunization record did not have the signature (including a rubber stamp or electronic signature) of the health-care professional who administered the vaccine.”
“The operation was not exiting the building during the fire drills in 3 minutes or less.”
“The operation was not checking the fire extinguisher monthly. This was corrected at inspection when the provider checked the fire extinguisher during inspection.”
“Two children had allergy medication at the operation that was not in the original packaging.”
“One classroom and the Chapel had electrical outlets with children present under the age of five that were uncovered. This was corrected at inspection when all electrical outlets were covered by staff.”
“Six staff files did not include training certificates for training that was completed.”
“Two children did not have a health statement in their record.”
“8 out of 8 childrens records did not have a date of enrollment listed on their enrollment forms”
“One caregiver did not have proper documentation showing education. Diploma was someone else's.”
