Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“It was determined a caregiver did not use good judgment when yelling at another staff member in front of children.”
“It was found that a caregiver yelled at the children as a form of discipline while in the classroom and on the playground.”
“Observed a dime size hole in the toddler classroom's wall.”
“A water hose which presented a tripping hazard was observed on the PreK outdoor space. This was corrected when the water hose was removed.”
“Based on the information obtained, it was determined that a caregiver was not demonstrating good judgement when playing with the children in care. This was corrected when the caregiver was removed from the operation.”
“Two contractors did not have orientation to the child-care center.”
“Two contractors were left alone with children in their careand counted in ratio who do not meet caregiver qualifications. A caregiver from the operation joined the group to supervise the children during the inspection so that the children were not alone with the contractors.”
“One caregiver did not know how many children she was responsible for during inspection. This was corrected when the number of children present was brought to the caregiver's attention and confirmed during inspection with the caregiver.”
“The loose-fill did not have the required depth when measured during inspection.”
“Three pieces of outdoor rocking and rotating equipment did not have enough use zone space when measured during inspection.”
“Three pieces of stationary outdoor equipment did not have enough use zone space when measured during inspection.”
“One caregiver had CPR training that was completed online.”
“Two children's immunization records did not have the signature (including a rubber stamp or electronic signature) of the health-care professional who administered the vaccine.”
“It was found during the inspection the operation had crib sheets on one crib and two play yards that were loose fitting. This was corrected at the inspection when the caregiver changed the fitted sheets.”
“It was found during the inspection children had access to the outside AC units.”
“A caregiver did not have proof of first aid certification and was alone with a group of children.”
“It was found that an operation did not report a positive Covid case to child care regulation. The case was reported to parents and staff.”
“5 of 10 infant feeding instructions reviewed had not been updated in the last 30 days.”
“A classroom with a specified age of 23 months had 10 children, being over ratio by one child. The oldest child was moved to the next age group to put the classroom into ratio.”
