Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A push toy was observed with shear cracks on either side of the push toy. NOTE: This was corrected when the push toy was removed.”
“One of the school-age children's files was missing school-age information. NOTE: This was corrected at inspection.”
“During the investigation, it was confirmed that a child ran out of the back door of the classroom that led to the playground, leaving a parent in the classroom with the remaining children.”
“One out of four files evaluated did not have the current immunization record available for review. Note: This issue was corrected during the inspection when the director was able to obtain the updated immunization record.”
“One of four files evaluated did not have the admission date on the child's enrollment form. Note: This was corrected at inspection when the date was entered by the director.”
“The director and a caregiver did not have current training in SIDS, Shaken Baby Syndrome, and Early Childhood Brain Development.”
“Some caregivers did not have current training in the following topics: (1) Emergency Preparedness (2) Preventing and controlling the spread of communicable diseases, including immunizations (3)Administering medication (4) Preventing and responding to emergencies due to food or an allergic reaction (5)Understanding building and physical premises safety (6)Handling, storing, and disposing of hazardous materials”
“7 out of the 8 children's files reviewed did not have a signed health statement from a physician on file although children had been enrolled for 12 months or longer.”

“The operation failed to submit the number of employees that have left their employment in 2023 by January 15th.”
“Loose-fill on playground was observed and measured less than 6 inches in depth.”
“Based on the information gathered, the operation did not report to Child Care Regulation an allegation that a child was hit by a caregiver.”
“Four out of the ten children's records evaluated did not have current immunization records on file.”
“The director's current CPR & First Aid certification was unavailable for review during inspection.”
“The electronic child safety alarm in van one is not functioning correctly and the left side door on van two is stuck and doesn't open.”

“One caregiver has an expired pediatric first aid and CPR certification.”
“The loose-fill measured less than 6 inches deep around the playscape outside.”
“The operation did not have active play space checklists available for review for the past 2 months. Only current motnh's checklist was reviewed.”
“A child was swaddled asleep in a crib. This was corrected at inspection when the caregiver unswaddled the child.”
