Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“There was not documentation of written feeding instructions for two infants in care that require them.”
“It was observed there were multiple holes in the fence in the ourdoor play area that need fixed.”
“There was not documentation available for safety drills and checking the detectors.”
“The primary caregiver is missing 10 clock hours for the last complete training year (10/26/24-10/26/25).”
“The primary caregiver was out of ratio during the investigation inspection as she was alone caring for two infants and eight children between 18 months old and 3 years old for a total of 10 children which was over by 3 children based on the ages of children she had in care. The primary caregiver acknowledged knowing she was out of ratio due to losing a staff member and not having any other help available at this time.”
“The operational policies do not include the required Parent Rights.”
“There was no documentation that the assistant caregiver has current and compliant first aid and CPR.”
“The operation was out of ratio and had more kids than they should have had based on the ages of the children in care. There were eleven children in care with one caregiver. There were three infants, seven children between 18 months and three years old, and one child 4 years old and up.”
“1 in 4 children's files evaluated did not have the child's immunization records in it even though the enrollment form stated they were attached.”
“2 of 3 staff files evaluated did not have documentation of meeting the educational requirement, the required affidavit, background check letter, and photo ID.”
“2 of 4 children's files evaluated did not have documentation of the required health statement in their files.”
