Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Medication was observed to no be in the original container with child's name.”
“The operation has not updated their operational policies to include parents rights or inclusive care statement.”
“The operation has not had a fire inspection completed since March of 2023.”
“The operation is missing the testing data for the inpact rating for the unitary surfacing.”
“The operation is missing the testing data for the inpact rating for the unitary surfacing.”
“The operation was found to not follow conditions of a background check for one individual.”
“I observed a group of 34 children playing on the big playground with a specified age group of 3. The max group for this age range is 30.”
“The operation is missing the testing data for the inpact rating for the unitary surfacing.”
“Caregivers use candy as a reard for "star sleepers", kids who sleep well during naptime.”
“The caregiver who administered first aid did not have a current certificate at the time of the incident.”
“3 people who reportedly worked directly with the caregiver, confirmed that a preschool teacher was humiliating children to redirect behavior.”
“This standard was part of an investigation. It was determined that the director knowingly allowed a caregiver to be present and in charge of children without having a complete background check on file and then with an ineligible letter.”
“This standard was part of an investigation. It was determined that a caregiver was present at the operation with an ineligible letter from the CBCU. The DOH was 12/1/2021, the ineligible letter was dated 2/9/2022, and the person was still allowed to work at the center through March 2022.”
“One caregiver was observed in a classroom with children prior to the operation receiving notification from the CBCU regarding the background check. The caregiver was removed from the classroom and sent home during the inspection.”
“This standard was part of an investigation. It was determined that an 11 month old non-walking infant was being placed on a cot for naptime rather than in a crib.”
“This standard was part of an investigation. It was determined that on March 9th and 10th, the infant room had 11 children, all ages 11m or younger. The exceeds the allowed group size by one child.”
“A child in care sustained an injury that required medical attention, and was not reported to Child Care Regulation.”
