Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A caregiver dropped a child off at a school they did not attend, and the child was left unsupervised.”
“The director didn't ensure that employees appropriately supervised children in care and as a result, one child was able to leave the daycare unnoticed, get on the daycare van and was transported to a school she didn't attend. The director didn't ensure the employee that transport the child adhered to policies and procedures.”
“A caregiver wasn't aware a child in care that she was transporting did not attend public school.”
“The monthly infant feeding sheets were not filled out and signed by a parent or physician.”
“The operational policy did not include information regarding allowing parents to observe the operation at any time durings hours of operation.”
“A review of the Employees Who Left Employment in 2022 Survey data from January 25, 2021, revealed that the center had not reported the number of employees who left employment in 2022 by January 15, 2023, as required.”

“The trash can in one of the toddler restrooms did not have a lid and contained soiled diapers. This was corrected at inspection when the trash can was replaced with a new trash can with a lid.”
“A caregiver was seen on video surveillance interacting with children inappropriately and not in a positive manner. The caregiver is no longer employed at the operation.”
