Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Two children did not have the required medication for food allergies ordered by a health care professional.”
“1 out of 10 children's records is missing admission information for the emergency medical authorization.”
“The operational policies in the parent handbook is missing Parent Rights.”
“1 out of 3 children with a diagnosed food allergy is missing Benadryl as indicated by the food allergy emergency plan.”
“During the inspection it was found that the operation did not have the current fire inspection posted. The posted inspection was dated 10/12/23 but the fire extinguishers show they were last serviced July 2024.”
“During the investigation the operation admitted to leaving the child unsupervised on the playground.”
“During the investigation the director admitted that she did not know she should report the incident to licensing.”
“It was found that the number of employees who ceased working at the operation in 2023 was not reported through the provider portal by 1/15/24.”
“It was found that an incident report was not created for the incident that occurred.”
“Based on the DFPS investigation completed, there is preponderance of evidence that physical abuse occurred.”
“Based on the DFPS investigation, camera footage was reviewed showing that the caregiver used corporal punishment when the caregiver hit a child on the face and flicked a child in the mouth.”
“Based on information during the investigation and interviews there is a sufficient amount of evidence to suggest that a caregiver did not demonstrate positive methods of discipline.”
“It was found that a child was unattended outside for 2 minutes.”
“A caregiver was missing their notarized affadavit form from their personnel file. NOTE: The deficiency was corrected at inspection.”
