Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Infants 12-18 months were observed engaging in screentime. NOTE: Corrected when the tablet was removed.”
“During the inspection, a 7 month old infant was observed in the room with a group of children who were watching Coco Melon on Netflix. NOTE: Corrected when the TV was turned off.”
“Verification was not available for review to validate: (1) the infant caregiver completed the required SIDS/Shaken Baby and Brain Development Training and (2) the caregiver who provided transportation on the date of the inspection completed the required transportation training.”
“An infant was observed in a swing with a propped bottle. NOTE: The director was informed of the deficiency during the inspection.”
“Based upon the results from the investigation, sufficient evidence was available to support an infant was in the same group with schoolers. A total of 14 children in care in three different groups: 8 children with 1 caregiver, 1 infant with 1 caregiver and 5 children with 1 caregiver, out of these 5 children, 4 were between the ages of 4-10 and the 5th child was 16 months old. The caregiver shared they had just finished feeding the child and was taking them back to their normal class. Corrected during the inspection when the child was back to their class.”
“During the inspection, handwashing procedures were not observed for a caregiver who failed to wash their hands and the hands of an infant following a diaper change.”
“Two infants were observed to be sitting in high chairs for approximately 10 minutes. The caregiver stated that the children had been in the high chairs approx.15 minutes prior to Licensing's arrival.”
“An infant was observed sitting in a high chair in the school-age area unspervised for approx.2-3 minutes.”
“The diaper changing mat was not sanitizied following a diaper change.”
“The fire extiguisher was observed on the floor of the front passenger seat and was not secured.”
“An infant was placed in a baby swing and the straps were not observed to be used. 2 children were observed in high chairs and straps were not used.”
“I observed a rotating cup piece of equipment on the playground with no use zone. This was corrected by pulling the piece into the center of the mulch surfacing. Photos were taken.”
“I observed a pile of trash blocking the walkway to the playground. Photos were taken.”
