Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Through investigation, it was determined that the operation served a child an identified allergen.”
“Through investigation, it was determined that staff did not communicate a parent's instructions for her child's meals, which resulted in the child eating food to which he was allergic.”
“Through investigation it was determined that a caregiver failed to ensure that a child with a food allergy was not served food containing that allergen.”
“Ten personnel files were observed to be without a Pre-Employment Affidavit for Applicants for Employment (Form 2912)..”
“One personnel file was observed to be without documentation showing how the employee meets age and education qualifications.”
“One child file was observed to be in need of updated immunization records.”
“A child was placed back into soiled clothing at the operation amongst other children and staff.”
“The director and a caregiver both stated a child was allowed to wear soiled clothing after a potty accident.”
“Information was observed to be missing from the medication logs for two children.”
“A Pre-Employment affidavit was not found in one employee's file.”
“A fire drill for October 2024 was not documented.”
“Personnel training records were not available for the 2021-2022 training year.”
