Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The operation did not have a current health inspection available for review at the time of inspection.”
“The operation did not have a signed current CPSC documentation form availalbe for review at inspection. This was corrected at inspection when the director reviewed the recall list and signed the form.”
“One child file was observed to have an expired immunization exemption affidavit.”
“This standard was tasked as part of an investigation and found deficient. A child was briefly left unsupervised on the playground.”

“This standard was evaluated as part of an investigation and found deficient. Candy is used a reward with children. Children who misbehave do not receive candy. Children who behave receive candy.”
“This standard was evaluated as part of an investigation and found deficient. A caregiver used aggression with children, forcibly grabbing and pulling children when misbehavior occurs.”
“Cleaning products were left out within children's reach. This was corrected at inspection when the cleaning products were moved to a higher shelf, out of children's reach.”
“Under the changing table, a door was left unlocked that contained diaper creams, scissors and a hole puncher. This was corrected at inspection when the supplies were locked in the cabinet.”
