Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The safety seats observed in two transportation vehicles were expired. Safety seats observed were manufactured ranged from between 2011-2015. One safety seat observed expired, 03/03/2018.”
“One out of two transportation vehicles observed did not have a working alarm system.”
“The pre-k/schoolers playground area loose fill was not maintained properly, in which dirt was exposed.”
“Two cribs in the infant classroom did not have snug fitting sheets during the walkthrough. One infant was observed napping in one out of the two cribs observed with loose fitting sheets. Sheets on the infant mattress were replaced during the inspection.”
“Several bags of mulch were observed on the pre-K playground area near the slide and underneath the play structure, posing a risk for a tripping hazard to children. While Licensing staff was present, the mulch bags were removed from children access.”
“The diaper changing surface in the infant classroom has multiple tears' therefore it is no longer non-absorbent.”
“Of the 8 personnel records evaluated, one has not completed 24 clock hours of pre-service training”

Procedures for parents to review and discuss concerns with the director 5. Procedures for parents to participate with the center's activities 6.
