Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A caregiver was observed alone with a group of children exceeding the classroom total for the youngest child in the group. this was corrected when a second caregiver returned to the room.”
“Two groups of children were left out of ratio when a caregiver left the cafeteria to let parents into the building, leaving one caregiver in charge of both groups. This was corrected during the inspection when the caregivers were notified that they both needed to be in the cafeteria as they were both counted in ratio.”
“Out of two staff files reviewed, one did not have training in communicable diseases, emergency preparedness, preventing food allergy reactions, understanding building and physical premises safety, and handling hazardous materials.”
“During the inspection, children were observed leaving the cafeteria and going to the restroom without caregiver supervision. This was corrected during the inspection when the children returned to the cafeteria and the caregivers were notified that children need to be supervised when they leave the cafeteria.”
“Out of the two staff files reviewed, one did not have proof of education on file and one did not have statements that she had received copies of the operational and personnel policies.”
“Out of the ten children's records reviewed, three were missing a date of admission and two children with diagnosed food allergies did not have allergy emergency plans on file.”
“Out of ten children's files read, two children had diagnosed food allergies and no food allergy plan on file.”
“Out of four personnel files read, three caregivers did not have current certification in First Aid and CPR.”
“Out of four personnel files read, two caregivers did not have education qualifications on file (HS diploma or equivalent).”
