Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on information obtained by DFPS, a caregiver stepped out of the classroom and left a group of children unsupervised on multiple occasions.”
“Based on information obtained by DFPS, play yards were placed right next to each other and touching, giving infants the ability to reach in.”
“A review of the Employees Who Left Employment in 2024 data revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.”
“The director was unable to provide proof of training completed in the most recent completed training year of 01/23 to 01/24.”
“A caregiver did not timely comfort an infant who was visibly upset.”
“An infant was in a restrictive device for longer than 15 minutes.”
“A 4 week old infant was observed asleep in a bouncer.”
“The most recent fire inspection was conducted on 2/9/23, more than 12 months ago.”
“A caregiver did not wash an infants hands after a diaper change.”
“Two children did not have immunization records on file.”
“There is sufficient evidence to determine that as of the inspection conducted on 1/18/24, one child's immunization record on file at the operation was out of date and did not contain all required immunizations.”
“The operation did not have documentation of a fire drill conducted in April.”
“One caregiver had CPR certification that did not include a hands-on portion as required.”
