Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Children were observed hanging on a jungle gym on the playground.”
“Two caregivers did not have current CPR/First aid training.”
“A 4 month old infant was sleeping in a restrictive device (baby bouncer).”
“The alarm on both buses were non-functioning.”
“The fire drills have not been practiced. The last fire drill recorded was in March 2023.”
“The cribs in the infant room (6w-12M) all had more than 1 mattress in each crib. The mattress instructions is to be used without a foundation.”
“Two out of 10 files reviewed did not have affidavits available for review during the visit.”
“Based on the information gathered during the investigation, it has been determined that a child was left unattended, the caregiver was not at a physical proximity that allowed visual and/or auditory awareness of the child in care.”
“The fire drills were not available to review. Last recorded drills were on August 2022.”
“One out of 10 files were lacking a notarized affidavit. Staff was hired on 3/2022.”
