Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“There was one caregiver in a room with a specified age group of 12-17 months with 6 children during naptime. Ratios for infants can never be reduced and the correct ratio for the specified age group is 1 caregiver to 5 children. This was corrected at inspection when a second caregiver was asked to come into the room.”
“Caregivers did not demonstrate good judgement when a child sustained an injury on the playground. Caregivers observed the child fall and catch themselves with both arms, and the child was crying and holding their arm after. Caregivers provided ice but did not alert management or seek another opinion on the injury.”
“Through an investigation, it was found that a caregiver hit a child with an open hand.”
“A parent was not notified of an injury that resulted in a fracture. The child fell asleep while icing their arm and when transitioning classes was holding the arm and crying.”
“This standard was found to be deficient when a caregiver admitted to using her personal cell phone while responsible for supervising children.”
“This standard was found to be deficient when a caregiver admitted to using her personal cell phone while responsible for supervising children.”
“4 staff personnel files were missing notarized employment affidavits.”
“A caregiver yelled at a child and used harsh language.”
“A caregiver pulled a child's arm and put the child harshly in the chair.”
“This standard was evaluated as a part of a DFPS investigation and was found to be deficient.”
“A caregiver who witnessed an incident that was suspected abuse, neglect did not report it.”
