Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“There was one classroom out of ratio at the inspection. There were 15 children in a classroom with 1 caregiver. The specified age group was 2 years old and the maximum ratio for this age is 1 caregiver to 11 children. This was corrected at inspection when the second caregiver came in from taking out the trash.”
“A child sustained an injury in care and the operation did not immediately notify the parent.”
“A caregiver did not demonstrate competency or good judgement when rough diapering resulted in a wound on a child and no documentation was completed prior to discovery by the parent.”
“A caregiver did not demonstrate competency by not doing a proper name to face per operational policies and as a result, a child was left unsupervised on the playground.”
“A child was left outside on the playground in the afternoon for approximately 6 minutes unattended.”
“3 staff hired after 09.01.2023 were missing a pre-employment affdavit. This was corrected at inspection.”
“Two children were missing special care needs information.”
“One caregiver was missing a notarized employment affidavit.”
“The operation did not report the number of employees who left the operation in 2023 to CCR.”
“This standard was found to be deficient as a caregiver was seen nudging and kicking a child who was lying on the floor after the child had refused to comply with the teacher's instructions.”
“This standard was found to be deficient after the operation failed to provide a fire inspection during the application inspection.”
