Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During an investigation it was found that a child was left unsupervised in a classroom at the operation.”
“Caregivers did not demonstrate competency when operation policy was not followed in regards to name-to-face checks. Caregivers missed two opportunities for a name-to-face.”
“This standard was found to be deficient. Two caregivers miscommunicated and 4 children were left unattended in a classroom for under 2 minutes.”
“This standard was found to be deficient.”
“Based on the investigation it was determined that a caregiver hit a child.”
“This standard was found to be deficient during the investigation.”
“This standard was evaluated during the investigation and was found to be deficient. A caregiver did not properly account for all children while transitioning a group of children to the playground, which resulted in a child being left unsupervised in a classroom for more than five minutes.”
“Based on a review of the information obtained during a DFPS investigation it was determined that a caregiver at the operation does not show patience or respect to the children in care. The caregiver is aggressive in her mannerisms and does not demonstrate courtesy to the children in care.”
“Based on a review of the information obtained during a DFPS investigation it was determined that a caregiver was observed, on two occasions, to have engaged in corporal punishment of children in care.”
“This standard was found to be deficient during the investigation as the caregiver admitted to not conducting thorough name-to-face roll call which resulted in a child being left alone and unsupervised in a classroom.”
“The operation failed to notify the child?s parents immediately when their child was left unattended while in care.”
“A child was left unsupervised in a hallway at the operation.”
“This standard was tasked over for this investigation and found to be deficient. The caregiver did not used good judgement when she didn't not ensuring that the bottle given to the infant was for the child by double checking the name, and performing a name to face check with the second caregiver present.”
“This standard was tasked over for this investigation and found to be deficient. During the course of this investigation it was determined that the caregiver mentioned in the allegation was observed and overheard yelling at children in care.”
“This standard was tasked over for this investigation and found to be deficient. During the course of this investigation it was determined that the caregiver named in the allegation was observed using a tobacco vaporizer while present in the classroom.”
“This standard was tasked over for the investigation and found to be deficient. The center was notified of a positive COVID case on Friday, but no report was entered into the Licensing system until Tuesday; as the director did not call and report the case to Intake directly. This put the reporting outside of 48 hour timeframe of reporting.”
