Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on the preponderance of evidence gathered, there is sufficient evidence to determine deficiencies related to this investigation regarding supervision. The caregiver did not intervene when the children were fighting at the basketball court.”
“Based on the preponderance of evidence gathered, there is sufficient evidence to determine deficiencies related to this investigation regarding the caregiver using an electronic device during care of children.”
“The training certificates in the personnel files did not have the current training certificates available for review.”
“The director was seen with a container of Fire Ant Killer that she had placed along an outter wall and sidewalk that lead to the school age playground. The owner indicated that the operation does have monthly extermination services from an external vendor.”
“Three children's records lacked the updated immunizations record for their age.”
“Based on the preponderance of evidence gathered, there is sufficient evidence to determine deficiencies related to this investigation regarding supervision. Children were seen engaging in different activities which included a child wandering in the classroom with no direction for an activity, children pulling, kicking, hitting, hopping on each other?s back while the caregiver answered the classroom telephone and when the caregiver was attending to other children sitting around the table with her back to the other children in the classroom. The caregiver was not able to intervene when necessary to ensure the children kicking and hitting each other ceased as the caregiver's back was to the children.”
“Based on the preponderance of evidence gathered, there is sufficient evidence to determine deficiencies related child/caregiver ratios specifically on 08/25/2025.”
“A caregiver counted in the child/caregiver ratio and accessible to children did not have an Eligible background check. Note: The caregiver left the operation during this inspection. The director was informed the caregiver cannot be present until an Eligible background is received from CBCU.”
