Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The 2912 form was not completed and was blank in one caregiver's file.”
“One of the caregivers did not have the 2985 affidavit form in her file.”

“According to the Texas Comptroller website, the business entity's Franchise Tax Account Status is Inactive/Forfeited. The operation is currently providing care to children with an inactive status.”
“The exit leading to the outdoor playground had a seventeen inch drop along the side of the walking path and exposed telephone wiring on the wall.”
“Play equipment (blue boat and four seated teeter totter ) on the playground did not meet the required use zone of 6 feet.”
“Evacuation diagrams were not posted in the classrooms.”
