Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“In the 10 children files viewed there were 4 files that did not have a Health Statement on file.”
“In the 10 employee files that were viewed there were 4 files that did not have an updated CPR and First Aid card.”
“In the 10 children files that were reviewed there were 3 files that did not have updated immunization records.”
“There was not an updated fire inspection available for review at the time of inspection. The last fire inspection was completed on 12/23/2024. Note: The operation has communicated and scheduled their next fire inspection.”
“In the 10 employee files that were viewed there were 4 out of the 10 employee files that did not have the employee's high school diploma. There were 4 out of the same 10 employee files that did not have the Pre Employment Affidavit. There were 4 out of the same 10 employee files that did not have the Notarized Affidavit.”
“There was cactus that overgrown to the operation's side of the fence in the outdoor play area. There was also mold that was forming in the AC units and vents in the classrooms.”
“There were two employees that were observed to be on their personal cell phones during the inspection.”
“There was a child that was out in the front desk area for about 2 minutes. Note: The child was seen by an employee, and taken back to the child's assigned classroom.”
“There was a tear that was noticed in one of the diaper changing mats, located in one of the classrooms. Note: This was corrected at inspection when the director replaced the torn diaper changing mat with a new mat.”
