Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A four-year-old child was combined with 15?17-month-old infants. This was corrected during the inspection when the 4-year-old was moved out of the older infant room after naptime.”
“Through evidence unrelated to incident, the operation used wall sits and running laps to discipline children's behavior.”
“A caregiver did not immediately intervene when an altercation occurred between children.”
“The operation did not update the child's food preference in the admission information.”
“An injured child who received medical treatment was not reported to licensing within the two-day required period after the incident.”
“One immunization record was not current.”
“One medication was expired.”
“9 Staff files were reviewed and all were missing the required Form 2985 Affidavit.”
“Based on information obtained through investigation a caregiver was assigned to a classroom without being trained on the requirements for a child with special care needs.”
“At the time of the inspection 1 child, with a noted food allergy on their enrollment form, did not have an allergy action plan from their physician on file.”
