Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The emergency evacuation diagrams that are posted in the classroom do not indicate the relocation center outside of the child care center or the shelter in place location(s).”
“Three children's allergy action plans were missing parent signatures.”
“There were several children in care with immunizations that are not current.”
“A toddler classroom had a small light on that didn't provide adequeate light in the classroom during naptime to allow for proper supervision. This was corrected at inspection when the caregiver turned on the bathroom light.”
“The Adventure Classroom had two outlets missing outlet covers. This was corrected at inspection when the director placed new outlet coverings on the outlets.”
“The staff annual training for storing of hazardous materials has not been completed by caregivers.”
“The playground for the primary classroom didn't have at least 6 inches of loose fill material around the slide and the climbing structure.”
“The operation did not follow label instructions in administering medication to an infant that required doctor instructions.”
“The primary playground support post were not marked to indicate the required depth of the loose-fill surfacing.”
“An investigation into this standard determined a violation. A child was left unsupervised in a classroom for over 20 minutes.”
“A violation of the standard was identified during an investigation when a child was left unsupervised in a classroom for over 20 minutes without staff knowledge.”
“Fire extinguishers are not inspected or recorded for the 2023 calendar year.”
“The outdoor space for the 2-3 year old playground did not have a useable secondary exit away from the building.”
“Medication administration training has not been recorded for a caregiver that administers medication.”
“Fire drills are not practiced on a monthly basis for the 2023 calendar year.”
“The emergency evacuation diagrams that are posted in the classroom do not indicate the relocation center outside of the child care center or the shelter in place location(s).”
“This standard was cited as part of an investigation and found deficient. A toddler was briefly left unsupervised on the playground equipment.”
“Of the 8 files reviewed during the inspection, there were 2 caregivers who have been at the center for more than 12 months and do not have any of the required 24 hours of training. The other caregivers are still within their 12 month timeframe.”
“A caregiver was present in a room with children before receiving notification from the CBCU that the caregiver is eligible, eligible with conditions, or provisionally eligible with conditions before allowing the subject to be present at your operation.”
“The director was allowing a caregiver to be present at the operation knowing they did not have a cleared background check from CBCU.”
