Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A caregiver roughly grabbed a 2-year-old child and sat the child on the ground harshly. The caregiver is no longer working for this operation.”
“This standard was found deficient as part of a DFPS Investigation. Based on information obtained during a DFPS investigation, the operation did not report suspected abuse and neglect with an incident at the operation after reviewing video footage.”

Based on information obtained during a DFPS investigation, the caregiver did not use good judgment when the caregiver informed the child?s parent of false information about how the child received the injury while in her care, although the operation was fully transparent with the parent. The operation terminated this caregiver.”
“The fire extinguisher in 1 of the 2 vans used to transport children was not secured.”
“5 out of 5 employees who transport children did not have current transportation training.”
“The playground did not have 6 inches of loose fill.”
“Based on information obtained during a DFPS investigation, a violation was found. A substitute caregiver did not have required affidavit in her file.”

“The operation does not have the proper depth of loose fill for the playstructure.”
“There is a saucer swing attached to a composite structure.”
“The operation hasnt conducted a checklist or walkthrough in two months. This is required at minimum monthly.”
“During naptime, it was observed that the mats were close together where the caregiver would have to step over children to get to another. This was corrected when the caregiver moved the children to create walk space.”
“The operation had less than two inches of loose fill. The tarp was showing also creating a tripping hazard.”
