Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The operation did not notify licensing (Child Care Regulation-CCR) in writing of a change in director within the required time frame. One of the directors no longer works for the operation since June of this year 2024.”
“The operation does not have a current gas leak inspection. The last inspection was completed on 4/8/2022.”
“During inspection, director was observed doing a transportation route. Director was not carrying the emergency medical transport and treatment authorization forms for the children being transported.”
“One caregiver was missing a total of 16 annual training hours of the required 24 hours of annual training.”
“The operation does not have a current annual verification of liability insurance. The operation has a next annual verification past due date of 2/07/2022. This was corrected at inspection after Director submitted a Verification of Liability Insurance in CLASS.”
“In the toddler room, one toddler did not have a labeled bottle/sippy cup. This was corrected on inspection after the bottle/sippy cup was labeled.”
“During inspection of the outdoor play area, a composite play structure with a slide had a crack on the slide. Also a water table was observed to be broken, exposing a pinch, and or shear point. Photos taken. This was corrected at inspection after both the slide portion of the play structure and the water table were removed from the playground. Photos taken.”
“The operation's fire marshall inspection is expired. The inspection was last conducted on 12/23/21.”
“One caregiver did not have pediatric first aid with rescue breathing.”
“One caregiver who was observed caring for toddlers did not have a current shaken baby/SIDS training. The caregiver's shaken baby/SIDS training expired on 10/27/22.”
“One child's file was missing the health statement.”
“Primary director's day care administrator's credential certificate expired on 11/30/22. The operation has not provided proof of a current credentialing certificate or letter of good standing from the credentialing organization/institution.”
“The operation does not have a current gas leak inspection. The last gas leak inspection was conducted on 9/26/19.”
“During inspection, during nap time in a central location, a few infants under 18 months were combined into one group with older children as old as 5 years of age. There were a total of 24 children in care. This was corrected at inspection after the nap time activity ended and the different age groups were re-arranged/re-located to their regular classrooms.”
“During inspection, during nap time in a central location, the operation was found to be out of ratio. One caregiver was found supervising a total of 24 children ages 12 months to 5 years with a specified age of 2 years. The most number of children one caregiver may supervise for the specified age group is 11 children. Child caregiver ratio cannot be reduced for children under 18 months during nap time. This was corrected at inspection after the nap time activity ended and the children were relocated/re-arranged to their regular classrooms.”
