Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A caregiver was observed being counted in ratio in the infant room without completing pre-service training.”
“Two different infants were observed sleeping in a restrictive device on two separate occasions for over 5 minutes each. *This was corrected at inspection when each child was removed from restrictive device.”
“The operation failed to notify parents of safe sleep deficiency within the required time frame.”
“A caregiver was observed feeding an infant. Caregivers eyes were observed closed and resting her head on her left shoulder.”
“The ceiling panel for one classroom was not replaced and continues with water stains. The other classrooms were observed with clean ceiling panels and the classroom previously with a loose ceiling panel was fixed.”
“Recite: A caregiver hired on 11/18/22 completed only 19 out 24 hours of Pre-service trainings.”

“There was no activity plan created for the month of May 2023 in the infant classroom. The last activity plan posted was for April 2023.”
