A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“The loose fill in the outdoor play area did not meet the 6 inch minimum requirement in any of the use zones for the play equipment.”
“The playground maintenance checklist was not documented since June 2025. The director completed the checklist during the inspection.”
“A caregiver with conditions on their background check stating they are not permitted to be alone with a group of children, was supervising five children alone. Another caregiver was put in the classroom during the inspection.”
