Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“an evacuation diagram was not posted in one classroom.”
“Nap mats were not labeled with name or which side faces down.”
“The director did not have a CPR First Aid certificate available for review.”
“A caregiver was present with children before an eligible background check was determined.”
“The operation did not have record that they conducted a fire drill in May 2024. The operation has no documentation that the fire extinguishers were checked in 2024.”
“One caregiver changed a child's diaper and did not wash their hands. This was brought to the caregiver's attention and the child's hand's were washed during inspection.”
“The operation's personnel files were not available for review by licensing during the inspection as they are not stored at the operation.”
“The operation did not have immunization records for one child in care.”
“The operation did not have a statement of the child's health from a health-care professional for two children in care.”
“Two caregivers completed online CPR training.”
“Outlet covers were observed to be missing covers.”
“safe sleep policies were not available for parents, as part of the operational policies.”
“The first aid kit contained expired items.”
“a trash can containing food or diapers was not coveered. A lid was provided for the trash can at the inspection”
“2 young children were sleeping in cribs with blankets. One had a blanket covering their body along with a mat and pillow and the other young child had a small stuff animal and blanket in the crib. This was corrected at Inspection when the director removed both blankets, stuff animal, and mat with pillow out of the crib.”
“There was no activity plan for toddlers.”
“The monthly inspection checklist was last completed on Nov. 22. This was corrected at inspection when the director completed the monthly checklist for this month and updated that her playground area is not in use at this time due to equipment that has fallen apart and needs to be put back together.”
“Documentation show that fire drills are not completed on a monthly basis and not within 3 minutes or less.”
“Out of 4 children files reviewed, 3 did not have release information or health statements. 2 out of 4 children files did not have emergency contact information or current immunizations.”
“Two caregivers are missing training in emergency preparedness, food allergies, building and physical premises safety, SIDS and shaken baby syndrome, and hazardous materials.”
“The Consumer Product Safety Commission (CPSC) recall list has not been reviewed since April 2019. This was corrected when the director reviewed the recall list and signed the The Consumer Product Safety Commission (CPSC) form.”
“The director is missing 2 out of the 30 required training hours. The director is missing training in emergency preparedness, food allergies, building and physical premises safety, SIDS and shaken baby syndrome, and hazardous materials.”
