A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“Blue Room playground for ages 4 and up was missing the required 6 inches for a 5 foot tall platform on the play structure.”
“10 children were missing special care needs information in their admission paperwork.”
