Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Of 10 student files reviewed, 2 were missing health statements.”
“A 3-year-old child was observed to be drinking a cold soda from a soda can while seated outside at the picnic table. This was corrected at inspection.”
“One caregiver who cares for children under 24 months of age had a recently expired Safe Sleep training.”
“The director was informed of a situation on 09/24/24 that a child had a vape and it was not reported.”
“The director was made aware that a child had a vape and parents were not notified.”
“A caregiver did not demonstrate good judgement by bringing a vape into the facility which went against the operation's policy.”
“A caregiver admitted to grabbing on a child's arm on at least two different occasions.”
“The toddlers classroom had a broken cubbie with a screw protruding.”
“Based on a review of information obtained during the DFPS investigation it was determined that although management had concerns regarding the management of the classroom by a caregiver, they did not provide adequate assistance to ensure the safety of children in care.”
“Based on a review of information obtained during a DFPS investigation it was determined that a child in care sustained injuries after being grabbed by a caregiver. The caregiver left scratches on the child's arm.”
“This standard was found to be deficient as the operation did not have a current Fire Inspection.”
“This standard was found to be deficient as two classrooms had entrances that were blocked by furniture. This was corrected during the inspection.”
“The operation notified the parents 7 days after receiving notification of a deficiency for standard 746.1201(4).”
“During a DFPS investigation, a caregiver was found Reason to Believe for Neglectful supervision. It was determined that a caregiver neglectfully supervised children in care after a child was left outside sitting under a slide and the caregiver left her head count tablet in the classroom and did not conduct a head count on any of her children.”
“This standard was found to be deficient as the caregiver transferred children to another class and failed to remove those children from her tablet and therefore did not have an accurate account of the children present in her group at one time. This resulted in a child being left on the playground unsupervised”
“This standard was found to be deficient as the caregiver did not ensure continuity of care of children when not communicating with another staff member clearly about which children were being transferred into the classroom which resulted in the caregiver not knowing which children she was responsible for.”
