Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Several outlets did not have covers in them. This was corrected when the director placed covers in the outlets that were missing them.”
“A group of children did not wash their hands before sitting down for lunch.”
“An infant was observed sleeping with a bottle. This was corrected during the inspection when the bottle was removed from the crib.”
“One caregiver was missing training in preventing and responding to allergic reactions.”
“An infant's hands were not washed after diapering. This was corrected during the inspection when the caregiver was reminded about handwashing and they washed the infant's hands.”
“The children's product certification form was last signed in December 2024. This was corrected during the inspection when a new form was signed.”
“Moods were not being added to the daily Procare reports for the older infant classes.”
“There was outdoor furniture in the back use zone of the swing set.”
“Three children needed updated immunization records, one four-year-old was missing a hearing screening, another four-year-old was missing a hearing and vision screening, and one child did not have an emergency contact on file.”
“One caregiver was missing proof of education and three were missing pre-employment affidavits.”
“Three caregivers were missing training in several of the required content topics.”
“The parent handbook was missing information on parent's rights and a statement that the operation is a gang-free zone.”
“During the investigation, it was found that the operation did not report to Child Care Regulation within two days when a child in care had a seizure and required medical treatment.”
“During the investigation, there was sufficient evidence found to determine that a child was left alone on the playground for an undetermined amount of time.”
“The written feeding instructions for three of the infants had not been updated monthly.”
“The infants' moods and daily activities were not being recorded in the Procare app. This was corrected during the inspection when the caregivers were shown how to add the mood and activities sections in Procare and began using them.”
“During the inspection infants/toddlers were observed eating lunch at a table without utilizing the manufacturer safety straps. This was corrected by the infants/toddlers being removed from the table.”
“During the inspection children were observed watching television in a room alone while caregiver was in different classroom preparing for nap time. This was corrected during the inspection by the caregiver returning to the classroom with the children.”
“Observed a caregiver responsible for children in care without a cleared background check. This was corrected during the inspection by the caregiver leaving the operation and the director submitting the background check.”
